This is a form used by the community (non-IT) to request an IT project.
Submitting this form will create a Service Request, which could eventually graduate to a formal Project Request and then to a Project.
1- At the Service Request stage, it can be tracked under Services > Ticket Requests
2- At the Project Request stage, it could be tracked under Services > Project Requests
3- At the Project stage, it could be tracked under Projects/Workspaces
